TCHD is accepting applications for a full-time, non-exempt RHTP Navigator to support Missouri’s Transformation of Rural Community Health Care (ToRCH Care) initiative. The RHTP Navigator will serve as a frontline, community-based member of the local rural health Hub and will be responsible for helping residents successfully connect to and complete needed clinical, behavioral, and social services.

This position will work across hospitals, clinics, behavioral health providers, pharmacies, emergency medical services, schools, public health agencies, and community-based organizations to improve access to services for rural residents. The RHTP Navigator will conduct screenings, manage referrals and follow-up, address barriers to care, maintain an assigned caseload, document activities in Hub-approved systems, and participate in community outreach and partner coordination.

The ideal candidate is compassionate, organized, self-motivated, and passionate about improving healthcare access and outcomes in rural communities. The successful candidate will possess strong communication, relationship-building, and problem-solving skills and will be comfortable working independently in community settings, managing multiple active cases, and collaborating with diverse healthcare and community partners.

Applicants must possess a high school diploma or equivalent and relevant experience in community health, care coordination, case management, patient navigation, public health outreach, social services, or a related field. Preference will be given to candidates with Community Health Worker training or certification, related post-secondary education, and experience serving Medicaid members, high-need populations, or rural communities. A valid driver’s license and the ability to travel routinely throughout the assigned service area are required.

TCHD provides a comprehensive benefits package which includes paid vacation (80 hours), holidays (9 Holiday and 3 Floating Holidays), and sick time (96 hours); LAGERS Retirement system (currently contributing 19.5 %) and 100% Paid Health Insurance for the employee.
